  4. Please to fill out the note section about what you are paying for your massage service, your copay balance, a gift certificate purchase, retail, or which belly dancing workshop, private lesson or series of classes.

    *This online payment option is excellent for gift certificate purchases. Be sure to let me know what information you would like written on the gift certificate and where to mail it.
